Participation Waiver and Release of Liability Agreement

I hereby release and waive my rights to sue Coach Dave Gleason, its officers, employees, coaches, volunteers, agents, representatives, and any other individuals associated with Coach Dave Gleason (collectively “Released Parties”) from any and all liability, claims, or demands arising from injury, loss, or damage to my person or property, whether caused by the negligence of the Released Parties or otherwise.

I agree to indemnify, hold harmless, and defend the Released Parties from any loss, liability, damage, or cost they may incur due to my participation in the Coach Dave Gleason activities, whether online, in-person, or otherwise.

I willingly and freely assume all risks associated with my participation in Coach Dave Gleason activities, including but not limited to, risk of injury, illness, and damage to my person or property. I understand the specific risks associated with training for improved strength, speed, power, endurance, and soccer skills training.

I consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during the activities.

I grant Coach Dave Gleason the right to take photographs or video footage of me during my participation in activities. I authorize Coach Dave Gleason to use such media for any lawful purpose, including but not limited to, publicity, illustration, advertising, and web content.

I agree to comply with all rules, regulations, and safety procedures established by Coach Dave Gleason for my well-being and to minimize the risk of injury or contraction of diseases.

If any clause or provision set forth in this waiver is determined to be illegal, invalid, or unenforceable under present or future law, then it shall be severable without affecting the enforceability of all remaining clauses or provisions.

I have read this waiver of liability, fully understand its terms, and understand that I am relinquishing substantial rights by signing it. I acknowledge that I am signing this agreement freely and voluntarily, and intend my signature to serve as confirmation of my complete and unconditional acceptance of the terms, conditions, and provisions set forth.

If the participant is a minor, I certify that I am the participant's parent or legal guardian and I make these representations and provide my agreement to this waiver and release on the participant's behalf.
